There are two ways to get to the heads on a Super Duty, and this is the choice that drives the hours line more than any other.
Cab-off means unbolting the cab from the frame and lifting it clear. It sounds drastic and it is the cheaper method at most competent diesel shops, because everything is then reachable from above with normal tools. Shops that do these routinely quote cab-off precisely because the hours are predictable.
Cab-on means working through the fender wells and from underneath, with specialty tools and considerable contortion. It is what many DIYers choose because they do not have a lift and a cab dolly, and it is what some shops offer when a customer objects to the phrase "we're taking the cab off." The E-Series vans are a separate case: there is no cab to remove, access is cab-over, and the job has its own hour profile. If your 6.0 is in an E-350 or E-450, do not accept an F-Series labor quote as applicable. Confirm van-specific hours with the shop directly.

The while-you-are-in-there list, and why it is nearly free
Here is the access-cost argument stated plainly. If the cab is off and the intake and turbo are out, the marginal labor to also replace the water pump, the thermostat, the turbo drain tube, and the STC fitting is small — minutes to an hour or so each, against a job already measured in tens of hours. If any one of those parts fails six months later, you pay the entire access bill a second time to reach a part that would have been trivial to change while you were there.
The items that earn their place on a 6.0 specifically:
- Updated STC fitting. The original snap-to-connect fitting on the high-pressure oil system is a known leak point, and a failed one presents as a hard-start or no-start that looks like injectors. Buried. Replace it.
- Turbo pedestal / drain tube update. A restricted drain pushes oil past turbo seals. Cheap part, deep location.
- Water pump and thermostat. The cooling system is already drained and open.
- Coolant filtration. Directly addresses why the first oil cooler died.
- 58V FICM. The module is heat-aged on every truck old enough to need this job, and a sagging FICM makes healthy injectors look sick.
- Glow plugs and harness inspection. Under the valve covers, which are already off.
Skipping all of these makes the invoice smaller and the truck's next two years worse.
What turns a bulletproof into an engine job
Sometimes the teardown reveals that the coolant intrusion already did structural damage. The cost swing is enormous, so ask your shop in advance what happens — and what it costs — if they find:
- A cracked or warped head. Machine shop, or replacement. This is the biggest single-line risk in the job.

- Damaged injector sleeves — coolant-side corrosion in the sleeve bores. Sleeve replacement is a specialist operation.
- Contaminated injectors. Coolant is not a lubricant. A set that ran on coolant-laced oil may be finished.
- A tired VGT turbo with a stuck unison ring, revealed once it is off the truck.
Get these on paper as a contingency schedule before the truck comes apart. A shop that will not price the contingencies is a shop that will price them later, when you have no leverage.
How to price your own 6.0 bulletproof job
- Confirm the diagnosis first. Do not bulletproof a truck on a hunch. Verify the coolant loss and combustion-gas path before spending an hour of labor.
- Decide cab-off or cab-on and get the hours quoted for that specific method, in writing, with the shop's rate stated separately.
- Choose your coverage tier — core items only, core plus recommended, or core plus recommended plus while-you-are-in-there — and price each tier as a separate list.
- Pick kit or à-la-carte. Compare the kit's contents against your tier list and price the gap parts individually.
- Add the fluids and consumables line. Coolant volume, flush chemicals, oil and filters, disposal.
- Add a machine-work contingency for head resurfacing or replacement, priced as a range, not assumed at zero.
- Add a diagnostic and reassembly-fault buffer. Something always needs one more part.

Do I need head studs, or can I reuse the head bolts?

Do I have to delete the EGR cooler?
No. An upgraded round-tube EGR cooler is a legitimate repair that keeps the emissions system intact, and it is what we recommend for any street-registered truck. EGR-delete hardware is sold as off-road-use-only, is excluded from our shopping feed for that reason, and removing or disabling an emissions control device on a road-going vehicle is regulated under federal and many state laws. Should I add a 58V FICM at the same time?
On a truck old enough to need bulletproofing, usually yes. The fuel injection control module ages in the same underhood heat that killed the coolers, and a module whose output has sagged below spec makes perfectly good injectors misfire, smoke, and hard-start.
