A gear on the crankshaft snout meshes with a larger gear on the camshaft, and the injection pump is turned off that same drive. Gears are heavier and noisier than a chain, but they do not stretch, they do not need a tensioner, and they do not have plastic guides that go brittle and shed pieces into the pan. That is why the engine was designed this way: a diesel injection pump has to stay in exact phase with the crankshaft under very high drive torque, and a gear train holds that relationship without drift.

Gear tooth wear and backlash. Gears wear on the tooth face over a very long service life, and as they do, the free play between the crank gear and cam gear increases. Excessive backlash shows up as noise and, in severe cases, as a timing relationship that drifts far enough to set correlation faults. Camshaft thrust plate wear. The thrust plate controls how far the camshaft can walk fore and aft.
