Hemi Tick — Diagnosis Flowchart and the Parts That Actually Fix It
TL;DR: "Hemi tick" is not one failure — it is three different noises that people give the same name. A sharp, metallic tick that is loudest on a cold start and fades as the engine warms is almost always broken exhaust manifold bolts, a cheap fix. A tick that is present hot, does not fade, and often arrives with a misfire code is a collapsed MDS lifter eating its roller and the cam lobe underneath it, which is an engine-out-of-service problem. A soft, even, sewing-machine ticking that has been there since the truck was new is injector and valvetrain noise, and it is normal. Sort which one you have before you buy a single part — the three fixes differ by an order of magnitude in cost and labor.
If you own a Gen III Hemi — the 5.7 Eagle, the 6.4 Apache/392, or any of the truck and car variants — you have heard the term and you have probably heard three contradictory answers about what it means. Forums will tell you it is manifold bolts. Your neighbor will tell you it is lifters. A shop will tell you it needs an engine. All three can be right, about different trucks.

The three noises, side by side
Almost every Hemi tick complaint resolves to one of these. The distinguishing evidence is when the noise appears and what it does as the engine warms.

The single most useful data point in that table is the cold/warm behavior, and it is free. Start the engine cold in a quiet place, listen, then let it reach operating temperature and listen again from the same spot. A tick that meaningfully quiets down when hot is a manifold-bolt tick until proven otherwise. A tick that is still there at full temperature deserves the rest of this article.
Why cold-start ticking usually means manifold bolts
The Gen III Hemi bolts its exhaust manifold to the head with a row of fasteners living in one of the harshest thermal environments on the engine. Every heat cycle stretches them. Over enough cycles, the outer bolts — usually the ones at the ends of the manifold, where thermal growth is greatest — fatigue and snap, sometimes flush with the head.
With one or two bolts gone, the manifold no longer seals flat against the port. On a cold engine everything is contracted and the gap is at its widest, so combustion pulses escape through it and you hear a crisp exhaust tick. As the engine heats, the manifold grows, the gap closes, and the noise fades. That thermal signature is the giveaway, and it is why this failure is so often misidentified as a lifter: people hear "ticking Hemi" and jump straight to the expensive answer.
How to confirm it without pulling anything apart:
- Cold-start the engine and locate the noise by ear, one side at a time, with the hood up.
- Look at the manifold-to-head joint with a light and a mirror. Broken bolts often show as a missing head, a witness mark of soot, or a black stain radiating from a port.
- Soot streaks on the head at the manifold flange are near-conclusive.
- If you have access, a smoke test or a mechanic's stethoscope on the flange will localize it to a single port.
The repair is cheap in parts and moderately annoying in labor — the broken shanks usually have to be extracted, and on some cylinders the head-side access is poor. What you need is a manifold bolt kit (the commonly referenced Dorman-style bolt-and-spreader-washer kits, or ARP hardware for a permanent answer), fresh manifold gaskets, and often replacement heat shields. When the tick is a collapsed MDS lifter
Chrysler's Multi-Displacement System deactivates four cylinders under light load by collapsing special lifters on those cylinders. It is the same architectural idea as GM's AFM, and it fails in the same way for the same reasons: the deactivation mechanism inside the lifter is a small hydraulic latch that must engage and release cleanly, thousands of times, on oil that is often overdue for a change.
When one of those latches sticks or the lifter's roller bearing lets go, two things follow in quick succession:
- The lifter stops following the lobe correctly. You hear a hard, deep tick that does not care about engine temperature.
- The roller that stopped rolling starts grinding, and it wipes the cam lobe underneath it. Once the lobe is gone, the valve that lobe operates stops opening, and you get a dead cylinder and a misfire code.
The window between "noise" and "wiped cam" can be short. That is the reason for the blunt advice: a hot tick with a misfire is not something to drive on while you save up.
Confirming it:
- Read codes first. A P0300-series misfire on a cylinder that MDS deactivates is strong supporting evidence. Which cylinders those are depends on your engine and calibration — it differs between 5.7 truck and car applications, so confirm for your exact year and engine.
- Listen with a stethoscope on each valve cover. A collapsed lifter is usually loudest over one bank and often one region of that bank.
- Pull the valve cover on the noisy side and watch valve motion at cranking. A rocker that does not move like its neighbors names the cylinder.
- Cut open the oil filter. Bronze or steel glitter means the roller bearing has already come apart, and the repair scope just grew.
What the repair actually includes. People price this as "a lifter." It is not. Getting to the lifters means the intake and heads come off on these engines, so the job is quoted as a set:

Shop the Hemi lifter category and Gen III Hemi camshafts together — buying them as one order is how you avoid discovering a missing part with the intake already on the bench. Springs, if you are changing the cam, are in valve springs.
The delete question. Many owners take the opportunity to delete MDS entirely: a non-MDS cam, standard lifters, solenoid blockoffs, and a calibration that stops the PCM from asking for cylinder deactivation. It removes the failure mode permanently rather than resetting its clock. It also requires a tune, and a tune carries its own considerations. The tick that is not a problem
A meaningful share of "Hemi tick" complaints are engines behaving exactly as designed. Gen III Hemis use solenoid fuel injectors that click audibly, plus a roller valvetrain that is simply not a quiet mechanism. Stand at the fender of a healthy 5.7 at idle and you will hear an even, light, sewing-machine patter.
Three tests that point to "normal":
- It has always done it. A noise present from the day the truck was new, unchanged for years, is a characteristic, not a failure.
- It is even. Injector noise is uniform across the engine. Mechanical failures are localized.
- Nothing else is wrong. No codes, no misfire, no oil consumption, no metal in the filter, normal oil pressure.
If all three hold, put the stethoscope away. The correct repair is no repair.
Diagnosis in order — the flowchart as a procedure
Run these steps in sequence. Stop at the first one that gives you an answer.
- Scan for codes, cold, before starting anything. A stored misfire changes the priority immediately and points you toward the lifter path.
- Cold-start and listen. Note whether the tick is present, where it is loudest, and how loud.
- Warm to operating temperature and listen again from the same position. Noise fades → manifold path. Noise unchanged or worse → lifter path.
- Inspect the manifold flanges on both banks for missing bolt heads and soot streaking.
- Stethoscope each valve cover and compare banks and regions.
- Cut the oil filter and inspect the media for metal. Any bearing material sends you straight to the full lifter-and-cam scope.
- Pull the noisy valve cover and watch rocker motion at cranking to name the cylinder.
- Only then buy parts — and buy the whole scope for the path you landed on, not one piece of it.
Steps 1 through 3 cost nothing but time and settle the great majority of cases. That is the whole point of doing them first.
Frequently asked questions
Is the Hemi tick always a lifter? No — and assuming it is, is the most expensive mistake in this diagnosis. Cold-start ticking that fades as the engine warms is far more often broken exhaust manifold bolts. Confirm the thermal behavior before pricing a lifter job.
Can I keep driving a Hemi with a tick? It depends entirely on which tick it is. A manifold-bolt tick is not urgent — annoying and eventually worse, but it will not destroy the engine. A hot tick from a collapsed lifter is destroying the camshaft while you drive, and continued driving turns a cam-and-lifter job into a short block.
Does the tick going away when the engine warms up mean it is fixed? No. It means the manifold sealed as it expanded, which is diagnostic information, not a repair. The bolts are still broken and the gap will be back on the next cold start.
Do I have to delete MDS when I replace lifters? No. You can install MDS lifters and an MDS cam and put the truck back to stock, which is the correct choice if you want a factory-configuration vehicle. Deleting removes the failure mode permanently but requires a calibration change. Both are legitimate; they are different trade-offs.
Should I replace the camshaft if only one lifter failed? Inspect it, then decide — but plan for yes. A lifter whose roller stopped rolling has almost certainly marked its lobe, and reusing a marked lobe under a new lifter is how the job gets done twice. With the intake and heads already off, the incremental cost of the cam is small against the labor of doing it again.
How do I tell a lifter tick from a rod knock? Frequency and pitch. A lifter ticks at roughly half crank speed with a light, sharp character from the top of the engine. A rod knock is a deeper, heavier thud that follows crank speed and comes from the bottom end, typically loudest under load or on decel. Metal in the oil with a bottom-end knock is a different and worse conversation.
Is a ticking Hemi worth buying used? Only at a price that assumes the worst case. Ask the seller to let you hear it cold and hot, scan it for stored misfires, and cut the filter if allowed. If they will not let you start it cold, walk.
